How do I find all my medical records?

If you are interested in getting a copy of your medical records, you will need to contact the doctor’s office, clinic, or hospital where you were treated.

How do I log into my banner?

Click the Login to Banner (SSB) link on the left side. Click the Log in to Banner link. In the User ID box, enter your Banner ID. In the PIN box, enter your DOB in MMDDYY format.

Can I request my medical records?

According to HIPAA, patients have the right to request their records. Other individuals can also request records on behalf of a patient. These include a parent, legal guardian, patient advocate or caregiver with written permission from the patient.

How do I find my banner ID?

You should see your Banner ID in the top right corner of the page, just before your name.

What is your Banner ID?

What is a Banner ID number? Everyone has a unique identifying number that starts with ‘900’. Per policy, it is required for password resets over the phone. Everyone has a Banner ID number and you will need to know it in order to get the help you need.

Can a doctor deny me my medical records?

Under HIPAA, they are required to provide you with a copy of your health information within 30 days of your request. A provider cannot deny you a copy of your records because you have not paid for the health services you have received.

Is Banner ID the same as student ID?

NOTE: Your Banner ID is the same as your student, faculty or employee number. Click on the link to retrieve your Banner ID number.

What is the banner ID?

The SID, sometimes referred to as the Banner ID or the ID number, is used to gain access to secure information (such as grades & student account records) and online procedures. Students are encouraged to protect their SIDs and to write the number down in a safe place for reference until it has been memorized.

Where do I find my banner ID UTSA?

Note: You can find your Banner ID on the front of your myUTSA ID card; it consists of several numbers preceded by an @ symbol. Provide a cellphone number or non-UTSA email address for Tech Café to send you a reset code and click “Submit.”
